Accident - an unexpected event happening suddenly and violently, with or without fault, and producing at the time objective symptoms of an injury.
Average monthly wage (AMW) - the lesser of the actual monthly wage received by an employee prior to his accident or illness, or the state maximum average monthly wage established each year for the state Employment Security Division.
C-4 Form - Employee's Claim for Compensation/Report of Initial Treatment. A copy of the form must be delivered to the insurer or third-party administrator. A copy of the form must be delivered to or the form must be filed by electronic transmission with the employer.

Compensation rate - 66 2/3 of the injured worker's average monthly wage, or 66 2/3 of the state maximum average monthly wage.
Functional Capacity Evaluation (FCE) - an evaluation by a physical therapist that lasts several hours, done for the purpose of giving the treating physician detailed information about the injured worker's ability to return to his or her pre-accident job duties and physical work restrictions.
Independent Medical Exam (IME) - an evaluation done by a physician chosen by the insurer to provide information about diagnosis, treatment recommendations, or work restrictions.
Maximum Medical Improvement (MMI) - a term used by the treating physician to indicate that no additional treatment is planned and that the injured worker is released from further care.
Permanent partial disability award (PPD) - the monetary settlement that is awarded to the injured worker at the conclusion of medical care for a permanent impairment, as defined by the AMA Guide to Evaluation of Permanent Impairment, (currently the 5th edition).
Physician Progress Report (PPR) - a form used by the treating physician after each office visit to show the diagnosis of any problems, any temporary or permanent work restrictions, and to indicate whether or not the patient is stable and ratable yet.
Physiatrist - a physical medicine specialist who coordinates the care of an injured worker by ordering any necessary physical therapy, any necessary diagnostic testing, or referrals to other medical specialists prior to giving any permanent work restrictions. This specialty should not be confused with a psychiatrist, who treats psychological problems.
Stable and ratable - a term used by the treating physician to indicate that the injured worker does not need any further medical care, and should be scheduled for an impairment evaluation (PPD evaluation) with an authorized rating doctor.
Temporary Partial Disability (TPD) - the benefits that are paid to ensure that the injured worker never receives less than his compensation rate while working a light duty position.
Temporary Total Disability (TTD) - the 66 2/3 of average monthly wage benefits that are paid when an injured worker is taken off work completely by his treating physician. These benefits are also paid when the injured worker has temporary work restrictions, but the employer cannot or will not provide work within the temporary work restrictions.
Third-party administrator (TPA) - a person or company hired by an insurer to manage its workers' compensation claims.
